We are currently seeking a motivated social worker to join us as part of a robust all-age adult mental health team, specifically focusing on streamlining hospital discharges. Your efforts will contribute to improving mental health services and outcomes for those leaving hospital settings. You will work with older and working-age adults diagnosed with functional or organic mental health issues, ensuring their smooth and secure discharge process.What you will do:
– Conduct comprehensive Care Act assessments.
– Develop person-centered support and discharge plans.
– Collaborate with carers, health professionals, advocates, and third-sector colleagues.
– Undertake Mental Capacity Assessments, chair Best Interests Meetings, and write Social Circumstances Reports.
– Participate in Mental Health Tribunals.
– Liaise with community resources to coordinate appropriate care and support.About You
