Pay Rate: £30 per hourJob Description:Liquid Personnel is recruiting Social Workers to join its client’s Adult Hospital Discharge team based in Manchester.What will your responsibilities be?
- Carry out Care Act assessments, strengths-based and person-centred, for citizens recently discharged from hospital.
- Complete timely reviews of hospital-arranged care packages, assessing for long-term support before transfer to community teams.
- Conduct Mental Capacity assessments, Best Interest decisions, and safeguarding work where required.
- Work independently in a hybrid model, seeking guidance for complex cases, with no duty function
- Great rate of pay £30 per hour

- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
