Riverside is a busy GP practice with a diverse patient list of approximately 9400 supported by a full complement of care navigators, GP's and a diverse allied health care team. This is a friendly and professional practice where you will be made to feel truly at home while making a real difference to patient care.Main duties of the jobThe post holder at Riverside GP Surgery is responsible for the delivery of high-quality general practice nursing to the entire practice population. The role focuses on providing evidence-based care for patients with long-term conditions, as well as delivering preventative health interventions across the practice.Working alongside a highly experienced nurse, the post holder will develop and refine their clinical skills, applying critical thinking and autonomous decision-making in patient management. The role includes supervising and supporting student nurses and working alongside experienced nursing staff to ensure high-quality patient care.Key Responsibilities:
  • Deliver holistic nursing care to patients with long-term conditions (e.g., diabetes, hypertension, COPD, asthma).
  • Provide preventative care, including immunisations, screening, and lifestyle advice.
  • Undertake clinical assessments, develop care plans, and make autonomous clinical decisions where appropriate.
  • Work collaboratively with GPs, pharmacists, and allied health professionals to coordinate patient care.
