The post holder will be a shift co-ordinator providing care for children, with a learning disability and associated challenging behaviour.
They will work as part of a multi-disciplinary team in delivering best quality care to the children who reside in the facility.What you'll do
Have shift co-ordinator responsibilities for a number of children, working in partnership with community disability teams, families, schools and other professionals and agencies.
- Ensure that robust systems are in place that capture the views of the young people to inform practice.
- Co-ordinate effective and holistic care in a multi-professional setting.
- Deliver and promote evidence-based practice and care plans.
- Contribute to the improvement of effective care using an outcomes approach.
- Lead by example in the promotion of high standards of care, to ensure a quality service. This will include supporting young people with physical and emotional care needs.
- Demonstrate commitment through regular attendance, efficient completion of duties and participation in department /team work activities.
- Monitor and report on client's progress and maintain accurate and up to date records.
- Make decisions based on professional knowledge and experience.
- Ensure the provision of an integrated quality service by maintaining effective communication with the multi-disciplinary and community services.
- Maintain good communication with patients/clients and relatives/carers ensuring that their individual needs are met, and address any issues which have been identified
