- Registered Nurse with valid registration with the Nursing and Midwifery Council
- Current CPR training
- Level 2 child protection
- Previous experience of working in a similar job role within Primary Care
- Knowledge of needs of patients with long-term conditions
- Awareness of accountability of own and others roles in a nurse led service
- Knowledge of health promotion strategies
- Awareness of clinical governance issues in primary care
- Knowledge of patient group directions and associated policy
- Change management skills and ability to support patients to change lifestyle
- Excellent communication and organizational skills.
- Empathy and sensitivity in patient interactions.
- Willingness and enthusiasm to undertake further training needs
- Proficiency in relevant IT systems for example, Microsoft Office, and clinical systems.
- Strong interpersonal skills.
- Motivated, forward thinking, adaptable, flexible, and able to work under pressure.
- A good understanding of the NHS and primary care landscape.
- Ability to work effectively with individuals and teams.
- Knowledge of public health issues in the area
- Awareness of local and national health policies
- Awareness of issues within the wider health economy
