An exciting opportunity has arisen for a band 6 Nurse Referral Advisor to be part of a multidisciplinary team based with the Single Point of Access (SPA) which is part of our Trusts commitment to deliver excellent and responsive assessment. The successful candidate will have the opportunity to develop within our evolving PCCT team , shape the vision of an inclusive Care Navigation Centre. We have been the first in the Midlands to develop a true Single Point of Access, with your help we would continue to be the first in many other endeavours.You will be working within a friendly team and be part of expanding a new and exciting service, incorporating streaming within the Emergency Department. Appropriate training will be given.Main duties of the jobProvide management and support for all patients to access appropriate care as dictated by their clinical requirements. This includes supported access to same day emergency care, admission or assessment our the identification of other community and alternative pathways. This is an integrated role to support all referring clinicians from a range of sources including GPs, clinicians, WMAS and NHS 111 referrals process and to improve the patients' hospital journey and improve the Emergency Department waiting time by reducing unnecessary Emergency Department attendances.You will need to have experience of enhanced nursing assessment, and an ability to communicate across a wide range of specialities and clinical groups. The role involves using your skills to direct patients to the most appropriate care facility using a clinical triage tool.Experience in ED is a requirement for this role.About usSandwell and West Birmingham NHS Trust (SWBT) is an integrated careorganisation with a budget of approximately £600 million and over 7,000 staff.   • The ability to organise and prioritise own workload within an agreed schedule of work and to work under pressure and within agreed timescales.
  • Experience of working as part of a team.
