– Manage and transfer communication support needs to other professionals and agencies as necessary.
– Conduct assessments and develop plans implementing evidence-based communication interventions for adults in long-term care.
– Collaborate with the multidisciplinary team, including occupational therapists and physiotherapists, to identify individuals who would benefit from direct specialist communication intervention.
– Carry out communication assessments, involving service users, their communication partners, and the care home team.
– Engage family members and the care team to enhance everyday communication skills and resources.
– Support communication within meaningful activities in the care home environment.Requirements:– HCPC registration is essential.
