We are looking to recruit a motivated, caring and enthusiastic Practice Nurse with excellent communication and interpersonal skills to join our friendly and supportive nursing team, 20-28 hours a week.You will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, working closely with the wider Practice Team to deliver the practice priorities.Main duties of the jobClinically, the focus of the role is management of patients with long-term conditions, treatment room duties and general nursing interventions for all patients. It is desirable you have knowledge of Diabetes Management, Asthma/COPD management, Hypertension, Family Planning/contraception, cervical cytology, long term condition reviews, wound care, childhood immunisations and vaccines.About usThe practice is an innovative, friendly, forward-thinking practice, working collaboratively within Farnham PCN. We have 3 Partners, 7 Salaried GPs, 1 Advanced Paramedic Practitioner, Practice Nurses, HCA and Clinical Pharmacists. List size 15,700. EMIS Web, AccuRX & Docman with Workflow Optimisation. We are part of Farnham Integrated Care Services working in collaboration with local practices offering same day access for urgent care along with MSK Practitioners and Home Visiting Services. X-ray, ultrasound and phlebotomy services available within Farnham Centre for Health. We are also a Training Practice.
