We are looking for a part time qualified nurse to work alongside our existing team to support the care of patients in Primary Care and in the GP Practice environment. There is scope for development and you will be working alongside our Lead Nurse Manager in delivery exceptional care and follow up to our patients.We can offer a great working environment, suitable supervision and an opportunity to grow and develop your skills.Main duties of the jobTo undertake standard nurse responsibilities, such as wound care, bloods, ECG, vaccinations, immunisations, INR's etc. Ensuring patient records are managed and updated on our clinical system, EMIS. To work alongside the existing team and deliver on key projects. Support Health promotion and Wellbeing programmes. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:a. To act as or supportthe Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice managerb. To produce or support the production ofthe IPC annual statement in conjunction with the practice managerc. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practiced. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practicee. To develop, implement and embed health promotion and wellbeing programmesf. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeingg. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients healthh. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSFi. As required, to diagnose and manage acute and chronic conditionsj. To prescribe medication for patients in accordance with PGDs and own scope of practicek. To review patient medications to enhance compliancel. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosagesm. To co-ordinate the provision of travel medicine servicesn.
