JOB SUMMARYThe role's main focus will be to provide high quality Planned and proactive care for care home patients in a weekly ward round; including but not limited to:- supporting the practice with ongoing management of care home patients by reviewing patients and amending Personalised Care & Support Plans and RESPECT forms.- supporting care home patients with anticipatory care discussions and planning.- providing acute medical treatment for care home patients with undifferentiated and undiagnosed problems- assessing and managing patients with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate.- Independently prescribing (where applicable to the role) and review appropriate medications in accordance with local policies and evidence based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence.- ordering investigations, follow-up results and arranging appropriate management including seeking advice where appropriate.- Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.- Supporting practices with ward rounds and MDTs.Main duties of the jobCLINICAL ASSESSMENT:To provide a first point of contact for care home patients with undifferentiated and undiagnosed health needs at the most appropriate time and place.Provide comprehensive and supportive end of life discussions with palliative care home residents, ensuring a thorough advanced care assessment and plan are developed.Complete frailty assessments and provide plans using this information.Provide and receive complex information; develops communication skills/present complex information to GPs where appropriate.To provide advanced assessment skills in history taking and clinical examination to formulate a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals, as necessary.Instigate and interpret diagnostic investigations as appropriate.About usAbout the PracticePractice Population of 13600 and we cover the town of Ledbury itself plus the surrounding villages and hamlets.5 Partner GP Team supported by multidisciplinary clinical team of Salaried GPs, Nurses, HCAs, Paramedic Practitioner, ANP, P, Pharmacists and Pharmacy Technician, First Contact Practitioner and Care Co-ordinators.Values: peer support, high quality patient centred care, innovation, continuous service improvement, compassion and good humour.Good CQC ratingTraining practice we have a combination of FY2s and trainees at different timesMedical system: EMIS Web, Docman 10, accuRxDetailsDate posted01 September 2025Pay schemeOtherSalaryDepending on experienceContractPermanentWorking patternPart-timeReference numberA4062-25-0002Job locationsMarket StreetLedburyHerefordshireHR8 2AQJob descriptionJob responsibilitiesWe are looking for an ANP or ACP to work 3 days per week carrying out ward rounds to our care homes.   • * The successful candidate will have or working towards a Post Graduate Degree Qualification in their field, Clinical Practice or equivalent qualification, with evidence of academic study and practical experience of situations that are typically encountered in a primary care setting.
  • * You will need to be registered with a professional body.
  • * It is a requirement for the candidate to have attained a post graduate qualification in Prescribing or be working towards qualification.
  • * To remain up to date with all mandatory training.
  • * To maintain up-to-date knowledge of safeguarding adult guidance to ensure that those at risks of abuse/suffering abuse are identified and the necessary process to protect them is followed.
