As a Care Coordinator, youll play a key role in ensuring that our clients receive consistent, personalised high-quality care. This is a dynamic position requiring excellent organisation, problem-solving, and communication skills within a fast-paced environment.Key Requirements:
  • Previous experience in a care coordination or similar administrative role within health or social care.
  • Strong IT skills and proficiency in Outlook, Word, and Excel.
  • Ability to prioritise tasks and manage competing demands under pressure.
  • Excellent communication and interpersonal skills.
  • A strong commitment to person-centred care and high standards of service.
Main duties of the job
  • Effectively coordinate and maintain Support Workers' rotas, ensuring continuity of care and appropriate client-staff matching.
  • Allocate fully trained staff in line with client-specific care requirements.
  • Respond efficiently to telephone and email enquiries.
  • Use care planning and rostering systems such as People Planner and Access Care Planning (ACP).
  • Liaise with clients, families, healthcare professionals, and colleagues to ensure seamless service delivery.
  • Respond to changes in care requirements or scheduling swiftly and professionally.
  • Provide regular support and supervision to the care team.
  • Participate in the on-call rota as part of a shared team responsibility.
  • Hold a valid UK driving license, access to a vehicle, and the ability to obtain business insurance.
