- To manage and update central information management systems within the Integrated Discharge Service
- Manage a central repository of information to support the Integrated Discharge Service in a timely and confidential manner.
- Update of referrals onto management information systems relating to patient details, needs and resources in a timely and accurate manner.
- Act as a central point for the Trust where live information can be gathered on capacity across the two sites.
- Produce and circulate robust daily information reports to support the Integrated Discharge Service processes and make available in a timely fashion to senior management colleagues.
- Actively develop effective internal and external relationships and work closely with colleagues to ensure a successful service delivery.
- Communicate with clinical and social care colleagues within the Integrated Discharge Service involved with each patient using a variety of communication methods including, verbal, written, interpersonal and computer, and passing information in a format that is appropriate to the individual request.
- Respond to external and internal telephone enquiries from staff and others in an efficient, tactful and helpful manner and ensure that any messages/information are passed to the appropriate person in a timely manner.
- Contribute to the overall effectiveness and development of the Integrated Discharge Service, the Division, the Trust and other partner organisations.
- Plan own workload in conjunction with Hub Manager to ensure continuity of service and support.
- To personally contribute to the overall effectiveness and development of the Integrated Discharge Service to ensure the delivery of safe and effective discharge processes.
- Take responsibility for ones own continuing professional development, including keeping up to date with activities of the wider Service.
- Maintain effective working relationships within Department and other officers of the Trust, in order to ensure delivery of professional service focused on achieving high level of patient satisfaction.
- Participate in any associated training in order to enhance knowledge and skills.
- To provide cover for colleagues during periods of leave, ensuring adequate cover to provide expected level of service, liaising with line managers as appropriate to ensure continuity of service.
- Support training for induction of any new Service members and support the identification of training needs and requirements.
- Undertake general clerical duties including photocopying, faxing and answering the telephone.
- To carry out other appropriate delegated duties as required by Hub Manager

- Good General Education (GCSEs)
- Business and Administration qualification or significant relevant experience
- NVQ 3 information/Administration
- Advanced IT Skills in MS Office applications (Word, Excel, Access, Outlook)
- Experience of using varied communication styles in an administrative setting
- Previous experience of management information systems (updating, extracting, and producing reports)
- Significant administrative experience working in an office/business setting
- Previous hospital or social care administration experience
- Use of Electronic Patient Record systems
- Knowledge of discharge pathways
- Good General Education (GCSEs)
- Business and Administration qualification or significant relevant experience
