Adults Social Worker | Reviews Team | Hybrid Working (2–3 Days Onsite)Nottingham City Council is looking for experienced and confident Adults Social Workers to join their fast-paced Reviews Team. This is a dynamic opportunity to contribute to high-quality adult social care reviews across a range of service user groups while supporting transformation and innovation across the authority.We’re seeking practitioners with strong assessment skills and a solid understanding of Direct Payment Misuse, Continuing Health Care (CHC), and a strengths-based approach to care planning.Key Responsibilities:Carry out reviews and reassessments across adult social careNavigate complex conversations and decision-making with service users and familiesIdentify misuse of direct payments and implement safeguarding protocolsApply CHC frameworks where relevantContribute to service transformation and cross-team supportMaintain accurate, timely records using Liquid LogicPractice Model:
