- Carrying out strengths-based assessments and delivering tailored care plans.
- Managing a varied caseload within a multi-disciplinary team.
- Building meaningful relationships with adults and their families.
- Offering support, guidance and advice to unqualified staff and students.
- Ensuring interventions deliver sustainable, positive outcomes.
- Applying your knowledge of legislation and best practices in adult care.
- Qualified Social Worker registered with Social Work England.
- 2-4 years' experience in Adult Social Care (assessment or long-term).
- Strong communication, mediation, and interpersonal skills.
- A creative and solution-focused approach.
- Confident in multi-agency working and risk assessment.
- A genuine passion for supporting adults and promoting independence.
