- Location: Integrated Care Home Service
- Conduct medication reviews for new care home residents who have been assessed by a specialist nurse practitioner.
- Attend a weekly MDT meeting every Tuesday afternoon with a consultant geriatrician, specialist nurses, and other healthcare professionals to discuss patient care and ongoing medical management.
- Apply up-to-date pharmacological knowledge to support effective medicine management for conditions commonly seen in the older population.
