
Primary Care Clinical Pharmacist

- Conduct clinical medication reviews for patients with multiple long-term conditions where medicines optimisation is needed, ensuring a patient-centred approach.
- Participate in multidisciplinary clinical meetings within the practice and Primary Care Cluster, LMC and Prescribing team within CTMUHB.
- Manage repeat prescription requests and queries within your scope of practice, and ensuring patients have appropriate monitoring and safety checks in place.
- Action changes to medicines responding to MHRA alerts, product withdrawals, supply shortages, and relevant local or national guidance.
- Maintain accurate and thorough records of all patient interactions, using appropriate clinical templates and coding in line with practice protocols.
- Take a proactive role in clinical audit and quality improvement initiatives, implementing recommendations where appropriate to enhance patient care.
- Provide patients with clear, independent information regarding their medications and any changes to their prescriptions, referring to other healthcare professionals for additional support when necessary.
- Monitor prescribing patterns within the practice against local health board guidance, making recommendations to prescribers or actioning changes within your scope of practice based on your findings.
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Independent Prescriber or currently working towards qualification.
- Minimum of 2 years experience as a pharmacist. Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice May hold or be working towards an independent prescribing qualification. Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
