– Screen and process all incoming referrals and communications effectively and efficiently.
– Address urgent needs, risks, and emergencies for ongoing cases, conducting unplanned reviews as necessary.
– Conduct Care Act assessments for new referrals that require thorough evaluation, working closely with residents and their carers in the community.
