  • Manage a caseload of complex mental health clients within the older adult service.
  • Provide integrated health and social care interventions, commissioning personalised care packages.
  • Conduct safeguarding investigations and continuously monitor clients’ mental states for signs of relapse and pathways to recovery.
  • Be an integral part of the Central AMHP team rota, undertaking assessments and coordination under the Mental Health Act.
  • Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
