Band 6 Community Psychiatric Nurse (CPN)

• Manage a large caseload of low intensity service users requiring minimal input, with crucial responsibilities such as administering injectable medication and regular check-ins every three months.
• Provide comprehensive care coordination for designated service users, working collaboratively with them and their carers using a supportive recovery approach.
• Facilitate tailored health and social care interventions, ensuring sensitivity to cultural needs and a focus on recovery and social inclusion.
• Assist in the administration of medication at the Lewisham Clozapine Clinic, actively participating on Tuesdays and Wednesdays, with training provided for the use of Point of Care equipment.
• Engage families and carers in the treatment process, offering direct interventions and psychoeducation where necessary.
• Collaborate with the multi-disciplinary team to assess, develop, implement, and evaluate high-quality, client-centred care packages.
• Maintain accurate clinical documentation and ensure effective communication with all stakeholders involved in the care process.
• Stay updated with current best practices, attend relevant training, and contribute to effective team dynamics.
