– Co-produce and review recovery and treatment plans that support and guide individuals through their recovery journey.
– Facilitate and promote progression from initial engagement through structured treatment to self-sustained recovery.
– Deliver expert knowledge, interventions, and support on substance misuse, including setting personalised recovery goals and harm minimisation strategies.
– Collaborate closely with GPs, social care professionals, family members, and significant others to secure the best possible outcome for service users.
