- We have a dedicated Director of AHPs, a Professional Lead OT and a Professional Lead Physiotherapist.

- Advanced rehabilitation & disability management skills
- Intermediate IT skills
- Knowledge of long-term conditions including end of life care and the drive toward care for these patients
- Knowledge and understanding of the NHS modernisation agenda and the implications for community services
- HCPC membership
- Degree/Diploma in Occupational Therapy Or Physiotherapy
- Relevant Professional Body Membership
- Evidence of continued professional development
- Post-graduate qualification relevant to the healthcare environment
- Extensive post-qualification experience in NHS or similar healthcare economy
- Substantial experience in a rehabilitation setting
- Experience in managing a complex individual & team caseload of patients
- Experience of supervising junior colleagues & students
- Experience in independently managing a caseload of patients
- Evidence of MDT working
- Previous community therapist experience
- Experience of leading a team
- Experience of case management including multi-agency working
- Performance management & dealing with complaints
