Are you a band 7 or an experienced band 6 looking to work in a dynamic and fast moving community rehab service?An exciting opportunity has arisen for a highly specialist OT/PT to be part of our Rehabilitation team which is part of Westminster CIS (Tri-borough service in Hammersmith and Fulham, Kensington and Chelsea and Westminster).You will be able to provide decisive and effective therapy input and will demonstrate leadership qualities, excellent communication skills and have a flexible approach when working under pressure. You will be passionate about the value of community rehabilitation and preventing hospital admissions.Main duties of the jobThis post will give opportunity to experience a wealth of learning and development. You will work alongside and show leadership in a responsive dynamic MDT who put high quality, patient centred care at the heart of what they do. We strive to provide first class, client-centred, goal focused therapy to patients in the community.The caseload will be split between community and Pathway 3, step down patients within nursing homes with support of MDT. Caseload is comprised predominantly of adults who may present with multiple complex health needs in community. As such, you will have the opportunity to develop a wide range of clinical skills and interests in areas such as discharge planning, falls and frailty, manual handling, orthopaedics, general medical and surgical conditions. An understanding of the management of multi-pathology patients and the impact that psychological/cognitive factors may have is desirable.This role will be based across Ellesmere and Forrester Court Care Homes and the office base is Hathaway House in W9.VaccinationThe successful applicant will have contact with patients or service users.
