This is a new role for a pharmacist to develop within the team to add some further skill mix and bring pharmacists into the service at an earlier stage in their career. This will be a test of change for a period of time with a view to develop a permanent position at a later stage.You will be responsible for the clinical pharmacy service to two designated independent hospices within the health board and will work alongside a specialist pharmacy technician and pharmacy support worker. Within the hospice, you will gain experience in the inpatient unit and with the wider multi-disciplinary team. The pharmacist role is already established and valued as a key member of the hospice MDT. The pharmacist is an essential member of the hospices Medicines Management Groups and provides advice to nursing and medical staff on all matters related to medicines.There will be the opportunity to participate in education and training for hospice staff and members of the primary care team such as ANPs and district nurses.Support for this post will be provided by the Macmillan Lead Pharmacist (Palliative Care) and an Advanced Pharmacist (Palliative Care).The postholder will contribute to, in conjunction with the Macmillan Lead Pharmacist (Palliative Care), in wider pieces of work which include the development and support of the already established Community Pharmacy Palliative Care Network.
