Warrington and Halton Teaching Hospitals is offering an exciting opportunity to recruit within the Acute Care Team.The team is highly regarded, supporting both medical and nursing staff in the care of the deteriorating patients and those transitioning from Intensive care to wards. This role encompasses critical care outreach, acute kidney injury specialist nursing and site management.We are looking for nurses with extensive experience with caring for critically ill patients who wish to seize the opportunity to develop themselves and the service, whilst being given formal and bedside training to support existing clinical expertise.This post is ideal for skilled practitioners who are passionate about patient safety and clinically effective care for patients at risk of deterioration and those post critical illness. Experience in clinical assessment and treatment of acutely ill patients in an acute setting is an essential for this post, as is a questioning nature and keen eye for detail.The successful candidates will be able to work well within a team and be willing to work a mixed shift pattern including days, weekends and nights.Main duties of the jobThe postholder will be responsible for
  • Attending clinical emergencies and providing care to deteriorating adult inpatients
  • Reviewing patients with acute kidney injuries, working closely with our renal consultants
  • Autonomously providing clinical review, including physical examination, assessment and diagnosis of deteriorating patients
  • Reviewing critical care patients post-discharge, co-ordinating MDT care to ensure opportunities for physical and emotional rehabilitation are maximised
  • Maintaining accurate patient documentation in-line with professionals body's requirements for all patients reviewed or where advice has been provided
  • Providing site management out of hours in conjunction with the off site on-call manager, being responsible for the allocation of staffing resources, managing incidents and supporting staff, patients and their relatives.
The post holder is required to have excellent interpersonal skills with ability to liaise effectively with the multidisciplinary team and act as an excellent role model, maintaining a positive workplace culture.
