- Conduct comprehensive assessments for individuals with complex physical, cognitive, or social needs
- Recommend and implement appropriate aids and adaptations to promote independence and safety
- Collaborate with service users, carers, and multidisciplinary professionals to develop tailored, person-centred support plans
- Deliver high-quality care in a community setting, ensuring dignity, respect, and empowerment at every step
- Qualified Occupational Therapist with current HCPC registration
- Proven experience in community-based work, particularly with aids and adaptations
- Strong understanding of complex health and social care conditions
- Excellent communication, interpersonal, and problem-solving skills
- Flexible working hours (to be agreed at interview)
