Hybrid Working | Adult Mental Health Review Team | Flexible Start DatesRochdale’s Adult Mental Health Service is seeking a highly experienced Adults Social Worker to join their dedicated Review Team. If you’re confident in mental health practice and have a background in both CMHT and Adult Social Care, this role offers the opportunity to work within a supportive, forward-thinking service.This is a full-time agency role offering hybrid working. The successful candidate will carry out targeted reviews and contribute to delivering high-quality, person-centred mental health care under the Care Act framework.Key Responsibilities:Carry out Care Act assessments and mental health reviewsUpdate and develop support plans in line with assessed needsApply safeguarding principles and the Mental Capacity ActWork collaboratively with CMHT professionals and health partnersMaintain timely and accurate case recordsPromote person-centred, strengths-based approaches in all interventionsPractice Model:
The Review Team plays a vital role in delivering effective care planning and intervention within Rochdale’s integrated health and social care model. The work is fast-paced but rewarding, focused on maintaining and improving outcomes for adults with mental health needs.Essential Requirements:Qualified Social Worker registered with Social Work EnglandMinimum 5 years’ post-qualified experience in adult social workPrevious experience in both CMHT and Adult Social Care settingsStrong understanding of the Care Act, MCA, and safeguarding proceduresAbility to work independently with a complex caseloadFull UK driving licence and access to a vehicleFamiliarity with Liquid Logic (desirable but not essential)Benefits:Up to £35 per hour (DOE)Flexible hybrid working modelStable and experienced management supportOpportunity to influence integrated care planningCollaborative and values-driven team cultureInterested?
