The Complex Care Needs Team in Wakefield is seeking to appoint a dedicated and compassionate Short Break Team Worker to join our dynamic and supportive team.Our team is responsible for assessing the needs of children across the Wakefield district who have special educational needs and/or disabilities (SEND), ensuring their social care needs are identified and appropriately met. We work collaboratively with a range of professionals to complete holistic assessments, identify unmet needs, and develop tailored recommendations. These may include signposting to relevant services or designing bespoke packages of support.In this role, the successful candidate will manage a caseload of short break cases for children with disabilities, under the guidance of the Team Leader, an experienced Enhanced Social Work Practitioner and Social Work Team Manager. Responsibilities will include maintaining and reviewing short break care plans, and ensuring that direct payments or alternative local authority provisions effectively meet the assessed needs of each child.The postholder will be expected to work as part of a multi-agency system, adhering to relevant policies and procedures, with a strong focus on achieving the best possible outcomes for children, young people, and their families.Flexibility and responsiveness are key to this role.
