Job Title: Patient Transfer Facilitator (Band 3)Pay: £12.21 - £20.63/hourLocation: Somerset, Priory Park, Wells BA5Working pattern: 7.5 hours, 5 days per week on a rotaJob Purpose:Post holder will be required to work their hours 5 days over a 7-day week to include bank holidays, weekends and shifts.To work closely with the Somerset Hub for Coordinating Care clinicians in facilitating a smooth referral process for service users and patients.Provide a single point of contact for information on patients to be transferred / admitted to community hospital beds within the Primary Care Trust.Co-ordinate the exchange of all information to the relevant wards, GPs and community professionals.Actively receive and manage waiting lists for transfer of patients, from District General Hospitals into community hospitals and GP admissions from the community. Provide trust-wide information on delayed discharges/transfers of care for reporting weekly at regional level.Main Duties:
  • Work with a team to manage bed availability across the Trust.
  • Share patient information between hospitals, GPs, and community teams.
  • Keep the Bed Management Database up to date with patients ready for transfer.
  • Help book patient transport and communicate with patients.
  • Track patient readiness for transfer and update records.
  • Provide weekly reports on delayed transfers of care.
  • Work with Senior Nurses to prioritise use of community hospital beds.
  • Follow Trust policies for patient admissions and transfers.
  • Support projects like booked admissions.
  • Help manage and maintain waiting lists.
  • Set up admin systems to support teamwork.
  • Support Somerset Hub clinicians with referrals and coordination.
  • Send out the weekly on-call rota and manage the on-call switch.
  • Forward fall referrals from the ambulance service to the right teams.
Qualifications & Skills Required:
  • European Computer Driving Licence (ECDL) or strong IT skills.
  • Confident using Microsoft Word, Outlook, and Excel.
  • Able to work independently and as part of a team.
  • Comfortable working in a fast-changing environment.
  • Able to meet deadlines and complete tasks on time.
  • Experience using IT systems for administrative or healthcare work.
  • Good level of English and literacy skills.
  • Willing to use technology to improve patient care.
