North Colchester PCN is made up of the following practices: Mill Road Surgery, Bluebell Surgery, Highwoods Surgery, Lawford Surgery and Winstree Medical Practice.We are looking for a person-centred individual who would like to work across all five of our surgeries supporting with all aspects of social prescribing on a part-time basis for 20 hours per week. This is a 6 month fixed term contract with the view to become permanent.Our social prescriber will play a pivotal role within our MDT (Multi-Disciplinary Team) by working closely with our PCN manager and care coordinating team. The successful candidate will link in with other members of the MDT where necessary working with a collaborative approach of connecting people to community groups and statutory services, for practical and emotional support.Main duties of the job
  • Providing personalised support to vulnerable individuals.
  • Working collaboratively with local organisations to support with positive lifestyle changes
  • Support meetings with organisations with the ability to manage and drive new initiatives
  • Providing information and advice about a wide range of services in order to promote independence within communities.
  • Creating strong links with local organisations, community and neighbourhood groups
  • Contributing to reducing demand on statutory services
  • Working with patients in the practice, or with patients in their own homes, to provide personalised person-centred support including care technology

  • Diploma Level 2 in Health and Social Care, or equivalent qualification or experience;
ExperienceEssential
  • Strong listening and communication skills.
  • Experience of working within healthcare, the voluntary and/or community sectors, supporting vulnerable groups.
  • You will be familiar with local resources and services and how to access them.
  • Motivated by helping people, with care and empathy.
  • Ability to deliver person-centred support in a non-judgmental way.
  • Experience in building relationships with key staff in GP practices , attending relevant meetings, becoming part of the wider network team, giving information and feedback on the role of social prescribing.
  • Understanding of the needs of small, volunteer-led community groups and the ability to support their development.
  • Commitment to collaborative working with all local agencies.
  • Experience and confidence in the delivery of lifestyle change interventions.
