What you will do:
  • Independently assess, treat, and plan discharge for patients within the community rehab team.
  • Evaluate and order necessary rehabilitation equipment with mindful reasoning.
  • Utilise recognised outcome measures to track patient progress.
  • Conduct family meetings to ensure cohesive support networks.
  • Maintain accurate documentation and patient records.
  • Collaborate with multidisciplinary teams to deliver comprehensive care.
Skills and Experience:
  • Previous experience in the NHS and post-qualification as an Occupational Therapist, ideally within community and rehab settings.
  • Strong understanding of outcome measures and patient-centred care.
  • Preferably a car driver with access to a personal vehicle for visiting patients.
  • Registered with HCPC with a minimum of 2 years of experience.
