What you will do:
  • Provide top-notch care to patients in their own homes, delivering a range of nursing services to support their health and recovery.
  • Manage and administer medications as needed, ensuring patients receive the correct dosage at the right time.
  • Work closely with a multidisciplinary team to develop, follow, and evaluate care plans tailored to each patient's needs.
  • Monitor patient progress, identify changes in condition, and respond appropriately to ensure the highest standard of care.
  • Engage and build strong relationships with patients and their families, offering support, education, and guidance throughout their treatment journey.
  • Maintain accurate records and documentation to keep the healthcare team informed and ensure continuity of care.
