- Conduct thorough mobility assessments and create targeted rehabilitation plans for patients.

- Take a lead role in discharge planning to ensure patients are thoroughly prepared and supported as they transition out of hospital care.
- Collaborate closely with the multidisciplinary team to deliver outstanding patient care and effective discharge strategies.
- Communicate effectively with patients and their families, ensuring they fully understand the treatment plans and discharge processes.
- You must be registered with the Health and Care Professions Council (HCPC) as a qualified Physiotherapist.
- A valid driving license is required as you may need to travel between sites and perform home visits.
