What you will do:
  • Provide dietary advice and intervention to individuals in community settings, helping them achieve their health and nutritional goals.
  • Assess nutritional needs and develop personalised care plans to support clients in managing their health conditions.
  • Collaborate with other healthcare professionals to ensure a holistic approach to patient care.
  • Conduct home visits and community clinics, utilising your own car to reach clients, therefore it is essential that you are a car driver with a valid licence.
  • Attend interdisciplinary team meetings to share insights and coordinate care strategies.
As a Band 6 Community Dietitian, you will need to be registered with the HCPC.
