The Inreach and Home Treatment Team, based in Chesterfield, is seeking a dedicated and enthusiastic Support Worker to join our welcoming and supportive team.This role offers a rewarding opportunity to work with older adults experiencing a deterioration in functional mental health. As part of a multidisciplinary team, you will contribute to delivering intensive, home-based treatment as an alternative to inpatient care. Under supervision, you will provide competent and compassionate care tailored to individual needs.The successful candidate will be required to travel across a wide geographical area, covering Chesterfield and North East Derbyshire, and therefore must hold a valid driving licence and have access to a vehicle.
