- NVQ level 3 or equivalent
- Evidence of continued professional development
- Knowledge of long term conditions
- Knowledge of mental illness and disability
- Knowledge of the cultures and religions of the area communities and to work across those cultures.
- Experience of rehabilitation in the community
- Demonstrates the ability to work with minimum supervision
- Ability to organise own time and workload
- Proven problem solving and clinical reasoning skills
- Ability to adapt communication style to suit different people, including use of verbal and non-verbal communication
- Physically capable of carrying out the duties required, including the manual handling of equipment or household items
