- Undertake both lone working and working within a team responsible for delivering an assessment, support and move on service to young people aged 18+ in supported accommodation.
- Support young people so that they can develop the skills and resilience to be confident and self-determining in their lives.
- Manage a caseload of young people, addressing their physical, emotional, social and behavioural needs.
- Running individual and group support sessions and other key activities within the supported accommodation to help them progress towards independence.
- Maintaining regular contact with the young people and recording interactions.
- Liaising with external organisations around the needs of young people.
- Contribute to maintaining a safe and welcoming environment, including preparing accommodation for new clients and supporting their transition.
- Completing risk assessments and support plans for young people.
- Excellent communication skills and commitment to the company values and ethos of the organisation.
- Experience in working with care leavers, young people or other vulnerable groups.
- Experience of Safeguarding in children and adults and operating safeguarding requirements and procedures.
- Experience and confidence of lone working.
- Strong literacy, numeracy and IT skills and ability to take notes.
- Ability to maintain professional boundaries, work collaboratively and reflect on practices for continuous development.
