Are you a Qualified Social Worker with a passion for supporting adults to live safely, independently and with dignity? We're looking for experienced practitioners to join a dedicated Adult Services Team covering both community work and hospital discharge.

- Carrying out strengths-based assessments and supporting timely, safe discharges from hospital.
- Managing a varied caseload, including complex care planning and safeguarding.
- Working closely with health professionals, families and voluntary services to provide joined-up support.
- Promoting independence, dignity, and wellbeing at every step.
- A Qualified Social Worker registered with Social Work England.
- 4+ years' post-qualified experience in adult social work settings.
- Strong knowledge of the Care Act 2014 and relevant adult safeguarding procedures.
- Skilled in building effective partnerships across health and social care.
- Confident, reflective, and committed to continuous improvement.
- A relevant social work qualification (DipSW, BA, MA, CQSW, etc.)
- Excellent communication and case recording skills.
- A proactive, flexible mindset - and the ability to hit the ground running.
