We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.An exciting opportunity has arisen for an Advanced Nurse Practitioner (ANP) or a trainee ANP to join our expanding frailty service at Glasgow Royal Infirmary (GRI). This is a key role within a multidisciplinary team, supporting the delivery of high- quality, person-centred care to older adults living with frailty.The Frailty Service in the North Sector is a dynamic and evolving model of care designed to meet the complex needs of older adults living with frailty. Based at Glasgow Royal Infirmary, the service integrates early identification using the HIS Frailty Assessment Tool, rapid access to Comprehensive Geriatric Assessment (CGA), and coordinated multidisciplinary input. The service has proudly demonstrated measurable improvements in patient outcomes, reduced length of stay and enhanced discharge planning. The team works closely with health and social care partners to deliver person-centred care, and is committed to continuous improvement, education, and innovation in frailty management.As an ANP, you will provide expert clinical assessment, diagnosis, and treatment planning including prescribing for patients with complex needs. The ANP role includes the authority to refer, admit and discharge patients. You will work autonomously and collaboratively, supporting early intervention, admission avoidance, and safe discharge planning.Key Responsibilities
  • Undertake advanced clinical assessments and formulate management plans.
  • Lead and contribute to Comprehensive Geriatric Assessments (CGA).
  • Support clinical decision-making and risk stratification.
  • Provide leadership and mentorship to nursing and support staff.
  • Contribute to service development, audit, and quality improvement.

About You
  • Registered Nurse with current NMC registration.
  • Proven experience in frailty, elderly care, or community nursing.
  • Applicants should have or be working towards, an appropriate Masters level qualification in Advanced Practice which provides evidence of competency in the following:
  • Clinical Examination & History Taking
  • Clinical Decision Making
  • Independent Prescribing - V300

We may also consider applications from experienced RN who possesses the commitment and willingness to undertake the ANP training pathway (Postgraduate Diploma over two years). Annexe 21 (Agenda for Change Terms and Conditions) will apply until successful completion of the ANP training programme.
