FULL TIME POST - 37.5 HOURS A WEEKAn exciting opportunity has arisen within the Colorectal Cancer Nurse Specialist team as a Cancer Care Coordinator. The role will involve supporting the Clinical Nurse Specialist team with the delivery of Personalised Care for Cancer.Personalised Care for Cancer includes supporting the provision of Holistic Needs Assessments, the development of a supportive care plan and offering Health & Wellbeing advice and support.In addition to this, the role will also involve supporting the implementation and administration of Personalised Stratified Follow-Up monitoring and tracking of surveillance tests and being a single point of access for patients. This will include rapid re-entry into the system for those people identified as having urgent or specialist needs.Main duties of the jobYou will work closely with all grades of medical and nursing staff and wider health care teams.
