– Collaborate with individuals who have complex needs to create support plans that enhance their abilities and help achieve their goals.
– Work jointly with different agencies and professionals to ensure the holistic needs of individuals are met.
– Manage a diverse caseload while safeguarding individuals, maintaining practice quality, and ensuring service compliance.
– Promote independence by embedding strength-based approaches in practice and support planning.
– Participate in and contribute to the development of Adult and Integrated Care Services, adapting to both local and national policy landscapes.
– Safeguard vulnerable adults, conducting investigations in line with South Tyneside safeguarding procedures.
– Provide support and guidance to less experienced staff, fostering an environment of learning and development.
