Vitalis are recruiting for an experienced Social Worker to work in a Hospital Discharge team in Rhondda Cynon Taf County Council.
Complex Discharge Assessments for D2RA Pathway 3, knowledge and skills in Mental Capacity Act and Continuing Health Care Funding required. Agile work between home and hospital sites. Full time only.
