You will work with our residents, family carers and commissioners to ensure that support packages comprehensively meet assessed needs, whilst also being innovative and cost-effective.Our social care staff within the hospital team work closely with NHS colleagues from acute and rehabilitation facilities, and with community health services, to facilitate safe, timely discharge. Their priority is to enable people to return to their own homes, utilising reablement and other services to enable them regain independence.The role offers a range of valuable experience.
