Job DescriptionThe Peat Road medical Practice is seeking an enthusiastic Advanced Nurse Practitioner for a newly created permanent ANP/APP post.We are an innovative and dynamic urban medical practice in Pollok Health Centre. Our practice is part of the impressive Pollok Civic Realm and houses a leisure centre (including a gym and swimming pool), modern Public Library, a Nursery, Citizen's Advice, a Stress Centre, a Community Museum and a Cafe. We are a Deep End practice and are lucky to have a full time Links Practitioner who helps connect patients to community resources and organises teaching events for staff to allow us to be familiar with all that is on offer for our patients within our community.You would be supported by 4 GP Partners, 1 Practice Nurse, 1 Prescribing Support Pharmacist and a Health Care Support Worker, and our experienced Admin team. We have an excellent District Nursing Team and Health Visitors.We have 5500 patients, we use Emis, Docman and Footfall electronic Triage, signposting and workflow optimisation to ease demand.The post holder will work as an expert clinical practitioner responsible for providing expert clinical care and management of both the stable and acutely unwell, making complex, autonomous decisions using advanced skills and knowledge. They will have overall responsibility for the assessment of care needs and the delivery of advanced nursing care, to a level equivalent to that of junior grade medical staff. The post holder will be an experienced practitioner, who acting within their professional boundaries will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
