  • Participate in home visits and complete Care Act Assessments to ensure all community care needs are determined.
  • Efficiently liaise with Scunthorpe General Hospital’s integrated discharge team to facilitate seamless patient discharges.
  • Conduct Mental Capacity Assessments and Best Interest Decisions where necessary to protect patient welfare.
  • Manage a caseload with varying complexity, providing tailored interventions that promote independence and well-being.
  • Collaborate with multi-disciplinary teams to identify innovative solutions for patient support and discharge.
  • Maintain comprehensive records following all regulatory requirements and best practices.
