This role will play a lead role in working in a complex environment on a range of operational, workforce, and service delivery initiatives across the primary care locality.The PCN Manager will provide strategic and operational support, ensuring effective delivery of the PCN Development Plan and supporting practices with the implementation of PCN priorities. They will establish approaches to decision-making and work with member practices to overcome challenges to implementation across the local population.Main duties of the jobThe PCN Manager will build strong relationships with a wide range of stakeholders across the locality to ensure collaborative approaches to service development and delivery. In doing so, they will support the integration of services across primary care, community services, social care, mental health, the voluntary sector and secondary care.This role is outward-looking to help secure resilience and sustainability by actively supporting business development, preparing proposals, and coordinating new service opportunities. The PCN Manager will also play a key role in recruiting, managing, and developing PCN staff, ensuring appropriate training, supervision, and workforce planning.About usFive Elms PCN was introduced in 2019 and has since become an integral part of Primary Care servicing its population of 43K patients.   • Educated to Degree level or able to demonstrate a period of previously related employment held in a senior management role within a service delivery setting, particularly in the Health Service.

  • A recognised qualification in Project Management.

  • Be able to speak confidently on a range of commissioning and primary care subjects, with the ability to provide definitive advice on national primary care regulation, policy and development.
Desirable
  • Able to demonstrate that you are a competent user of the whole Microsoft Office group of applications.
ExperienceEssential
  • Experience of developing primary care provision at scale, for example working with a federation or collaboration of general practices
Desirable
  • Have an understanding of the national policy direction for primary, community and secondary care as evidenced in the Five Year Forward Review together with good practical and conceptual knowledge of healthcare improvement methods and practices
