– Collaborate within a multi-disciplinary team to ensure seamless hospital discharges and support plans.
– Conduct comprehensive care and support assessments tailored to individual needs.
– Undertake mental capacity and Best Interest’s assessments to safeguard client interests.
– Engage in vital safeguarding work to protect vulnerable individuals.
– Work with the transfer of care hub, prioritising and triaging for efficient patient support.
– Develop and review reablement plans to aid in the recovery and rehabilitation process.
– Provide essential support to carers, recognising their crucial role in the care process.
– Participate actively in decision support tool meetings to ensure comprehensive care planning.Requirements of the Social Worker:
– A degree or equivalent qualification in social work is essential.
– Current registration with Social Work England is required.
