We are currently looking for a temporary part-time Care Navigator to join our friendly team. Hours of work will be as follows:Monday - Friday 1.00pm - 5.30pm (22 5 hours per week)Are you empathetic, caring, kind and people focused, yet resilient and committed? If so, we would love to hear from you.As this role is a temporary role to cover maternity leave we would on this occasion prefer candidates to have General Practice/ SystmOne experience.Main duties of the jobSuitable candidates will have excellent administration skills, be organised and have excellent communication skills and telephone manner. The role requires flexibility, willing to cover leave and to work as part of a team.Main aspect of the role is to be the first point of contact for patients, secondary care and outside agencies. You will need a positive attitude. You will be required to provide admin support to our Clinicians by processing various tasks including prescriptions and follow up appointments whilst adhering to Practice policies and maintaining confidentiality at all times.About usBradford Student Health Service is a busy GP practice in Bradford with approximately 10,500 registered patients. We are a University practice situated in close proximity to the University of Bradford.Care navigation is an integral part of the practice and is the patients first line of communication for all their health care needs. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:a. Maintaining and monitoring the practice appointment systemb. Process personal, telephone and e-requests for appointmentsc. Answer incoming phone calls, transferring calls or dealing with the callers request appropriatelyd. Signpost patients to the correct servicee. Initiating contact with and responding to, requests from patients, team members and external agenciesf. Read code data on SystmOneg. Photocopy documentation as requiredh. Data entry of new and temporary registrations and relevant patient information as requiredi. Input data into the patients healthcare records as necessaryj. Direct requests for information i.e. SAR, insurance / solicitors letters and DVLA forms to the administrative teamk. Manage all queries as necessary in an efficient mannerl. Carry out system searches as requestedm. Maintain a clean, tidy, effective working area at all timesn. Monitor and maintain the reception area and notice boardso.
