The Manchester Sickle Cell and Thalassemia Service is a city-wide service and it part of Adult and Specialists Services the Central locality.Haemoglobinopathy Nurse Counsellor-Band 6 ,to join a team ,that provide support for patients with Sickle Cell Disorder.Providing community support ,attending Consultant clinics for patients discharged from the Sickle Cell Unit/Ward 10 ,MRI. Supporting patients in the home following discharge.Main duties of the jobHaemoglobinopathy Nurse Counsellor will be part of community nurses working with SCU at MRI. Attend sickle clinic for patients discharged from Ward 10/ SCU, support and follow up with community visits and specialist adult nursing in community.Provide health awareness and self care management.Establish an education and health support facility for adults affected and families/carers and provide continued care and education, support and advocacy. Post holder will play important role in ensuring delivery of equitable City-wide Haemoglobinopathy service provision and primarily be responsible for providing life span personalised care and advice on haemoglobinopathies within community setting as part of multi-disciplinary team. Post holder will play active part in delivering adult services, ensuring high quality, patient-centered, safe, efficient care in line with Standards for Management of Sickle Cell disease. Be a professional role model, leading and coordinating care of young people/adults with Haemoglobinopathies and rare anaemias. Take continuing responsibility and accountability for nursing management and organisation of patient group. Have extensive knowledge of Sickle cell and Thalassaemia. Provide support for patients/families, teaching for staff on inpatient and day care units, ensuring a comprehensive service with a multidisciplinary team approach.
