Salary Available Upon Request30 hours a weekAre you an experienced practice nurse, or a nurse in another discipline seeking to move into general practice? If so, now is the time and we would really like to hear from you with a view to joining our friendly and highly skilled multidisciplinary team.As a treatment room nurse, we would be looking to provide a full range of nursing services. Competencies including INR dosing, childhood immunisations, smear taking, complex dressings, travel vaccinations etc.Training and development can be provided for less experienced nurses, Including an orientation/induction, supernumerary time shadowing one of our other nurses, clinical mentorship, and support from the entire Multidisciplinary team. However, experience would be advantageous.We have two other nurses providing Chronic Disease Management, however there is potential for further training when established.A competitive salary is offered. Work would be split between our main site and our branch site, so the ability to travel between sites in a timely manner is essential.Main duties of the jobClinical practiceAssess, plan, develop, implement and evaluate treatment programmes that promote health and well-being.Assess, plan, implement and evaluate individual treatment plans for patients with a known chronic condition.Proactively identify and manage treatment plans for patients at risk of developing a chronic condition as appropriate.Work autonomously and with other health care professionals to diagnose, monitor, manage and treat chronic conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and healthcare service delivery.Support patients to adopt health promotion strategies that promote principles of self-care.Implement and participate in vaccination and immunisation programmes (adults and children).Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on individuals health and well-being.Implement and evaluate individual treatment plans for patients with a known chronic condition.Support patients to adopt health promotion strategies that promote patients to live healthily and encourage principles of self-care.Assess and care for patients presenting with both complicated and uncomplicated wounds.Advise, support, screen, implement and participate in vaccination and immunisation programmes for patients.About usNant Dowlais Health Centre is an established GP Practice serving a 10,000-patient list in West Cwmbran, led by a Multidisciplinary partnership.This is an exciting time to join Nant Dowlais Health Centre, as we strive to develop our service using quality improvement methodology.
