- Conducting patient reviews and assessments for long-term conditions.
- Developing and implementing individualised care plans.
- Providing education and support to patients and their families to promote self-management.
- Administering and monitoring treatment plans in line with clinical guidelines.
- Collaborating with GPs, healthcare assistants, and other multidisciplinary team members.
- Keeping accurate records and ensuring compliance with NHS policies and procedures.
- Five years experience of working in primary care.
- Experience with clinical risk management.
- Routine Practice Nurse competences - Smears, Immunisations
- Experience with audit and able to lead audit programmes
- Registered Nurse - NMC Qualification.
- Specialist experience and qualification in Asthma, COPD, and Diabetes Management.
- Hold an Independent Prescribing Qualification
- Knowledge of relevant clinical guidelines and NHS quality frameworks.
- Knowledge of SystmOne
- Excellent communication and patient education skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Five years experience of working in primary care.
