- As a Hospital Discharge Social Worker, you will play a crucial role in ensuring that patients receive comprehensive support and resources to transition successfully from hospitalisation to their homes or alternative care settings. Your responsibilities will include:
- Conducting comprehensive assessments of patients' psychosocial needs, including evaluating their support systems, living arrangements, and access to community resources.
- Collaborating with interdisciplinary healthcare teams to develop individualised discharge plans that address patients' medical, emotional, and social needs.
- Providing counselling and support to patients and their families to facilitate adjustment to changes in health status, care needs, and living arrangements.
- Coordinating with community agencies, social services, and other healthcare providers to arrange post-discharge services, such as home health care, rehabilitation, or long-term care.
- Advocating for patients' rights and ensuring that their preferences and concerns are addressed throughout the discharge process.
- Documenting all assessments, interventions, and referrals in accordance with professional standards and organisational policies.
- Social Worker qualification
- Registered with SSSC as a Social Worker
- Car driver
